"It's not about handing out cash to people,"

In an article about refundable tax credits in the WSJ, there is this:

"The McCain campaign rejected charges of a double standard because Sen. McCain's refundable tax credits would have to be spent on health care. "It's a very different operation. It's not about handing out cash to people," said senior campaign adviser Douglas Holtz-Eakin."

If it's not being sent to people, who is it being sent to? All he's saying is that it's targeted, but, then, all refundable tax credits are targeted for something or some reason. Can someone in the McCain camp talk plainly about economics?
